Thyroid Diet for Hypothyroidism: Foods and Timing
A practical hypothyroidism diet: what to eat, iodine and selenium facts, when to take levothyroxine around food, soy and cabbage myths, and a sample day.

There's no special diet that fixes hypothyroidism; levothyroxine treats it. The best thyroid diet is a normal balanced one: vegetables, whole grains, pulses, lean protein and dairy, with iodised salt in normal amounts. What matters most is timing: take your tablet on an empty stomach and keep calcium, iron and soy away from it.

Can food treat an underactive thyroid?
No. Hypothyroidism means the thyroid makes too little hormone, and the standard treatment is a daily levothyroxine tablet, as the NHS and the American Thyroid Association explain. Diet supports your general health, weight and energy, but it can't replace the medicine.
How should you time food and your thyroid tablet?
- Take levothyroxine with water on an empty stomach, 30 to 60 minutes before breakfast, or at bedtime at least 3 to 4 hours after your last meal.
- Keep calcium supplements, iron tablets, antacids and multivitamins with minerals at least 4 hours away.
- Coffee, soy milk and very high-fibre breakfasts right after the tablet can reduce absorption. Wait the full time.
- Take it the same way every day, so your dose stays reliable.
What about iodine and selenium?
Iodine is needed to make thyroid hormone, but most people get enough from iodised salt, dairy, eggs and fish. Extra iodine supplements are not recommended for most people with hypothyroidism, and too much can be harmful. Selenium (found in eggs, fish and a few Brazil nuts) is part of a normal diet; don't take high-dose supplements without advice.
Are cabbage, broccoli and soy bad for the thyroid?
These are common myths. Cooked cabbage, cauliflower, broccoli and soy foods are fine in normal amounts. Very large amounts of raw cruciferous vegetables, or soy taken right next to your tablet, are the only real concerns.
What should you eat?
Eat: vegetables and salads, whole grains (whole-wheat roti, oats, brown rice), daal and chana, eggs, fish, chicken, low-fat dairy, fruit, nuts and seeds, iodised salt in normal amounts.
Limit: sugary foods and refined snacks, fried food and large portions, which matter more because hypothyroidism can slow metabolism and cause weight gain.
Avoid: high-dose iodine or kelp supplements, and "thyroid support" herbal products unless your doctor approves.
What is a sample day for hypothyroidism?
- On waking: levothyroxine with a glass of water
- Breakfast (45 minutes later): vegetable omelette with 1 whole-wheat roti, or oats with milk; tea or coffee
- Mid-morning: a guava or apple
- Lunch: 2 roti, daal, mixed sabzi (cooked cabbage or cauliflower is fine), salad, dahi
- Snack: a few almonds or roasted chana
- Dinner: fish or chicken curry with ¾ cup rice and vegetables
What about weight and tiredness?
When thyroid levels are low, weight can creep up and energy drops. Once your levels are right, a normal healthy diet and regular activity usually help. Use the calorie calculator for a sensible target and the BMI calculator to track progress.
How do you know your dose is right?
Your doctor checks TSH, sometimes with T4, every few months at first. You can understand your results with the thyroid test checker, then discuss them at your next appointment.
When should you see a doctor?
See your doctor if you feel very tired, cold, have constipation or weight gain despite treatment, or have palpitations, shaking or weight loss, which can be signs your dose is too high. Pregnancy changes thyroid needs, so tell your doctor as soon as you're pregnant.
Is a gluten-free diet good for hypothyroidism?
Only if you also have coeliac disease, which is more common in people with autoimmune thyroid disease. If you have symptoms like diarrhoea, bloating or anaemia, ask your doctor for a coeliac test before cutting out gluten, because the test needs you to still be eating it.
What are good snacks for hypothyroidism?
- A piece of fruit with a few nuts
- Plain yogurt or a glass of milk (taken at least 4 hours away from your tablet)
- Roasted chana
- A boiled egg
- Vegetable sticks with hummus
Does a thyroid diet help with weight loss?
Once your thyroid levels are controlled, weight follows the same rules as for anyone else: a modest calorie deficit, plenty of protein and fibre, regular activity and good sleep. Untreated or under-treated hypothyroidism makes this harder, so keep up with your blood tests.

Frequently asked questions
What should I avoid when taking thyroid medicine?
Take levothyroxine on an empty stomach, usually 30 to 60 minutes before breakfast, and keep calcium, iron supplements and antacids at least 4 hours apart. Coffee, soy and high-fibre meals right after the tablet can also reduce absorption.
Are cabbage and broccoli bad for hypothyroidism?
In normal cooked amounts they are fine for most people. Only very large amounts of raw cruciferous vegetables are a concern, especially with low iodine intake.
Should I take iodine supplements for hypothyroidism?
Usually no. Most hypothyroidism in iodine-sufficient countries is caused by autoimmune thyroiditis, and extra iodine can sometimes make thyroid problems worse. Use iodised salt normally and ask your doctor before taking supplements.
